After finishing mesial vertical bone cut with osteotomes (Fig.16), the distal mucoperiosteal flap (F in Fig.17) is pushed mesially. An osteotome is placed between the implant and #11 to make the second vertical bone cut.  Note the left finger is placed palatally while the osteotome is being tapped for protection.
