The distal root of the lower right 2nd molar is not only close to the inferior alveolar canal (N), but also to the lingual concavity (Fig.17, preop CBCT coronal section, 47-year-old Caucasian lady). A 7x11 mm implant is placed immediately post extraction (Fig.18). The beauty of the immediate implant is achievement of high insertion torque (using osteotomes for osteotomy) without violating the adjacent important anatomic structures. The lingual plate may have undergone a green-stick fracture in a small area.
