Buccal Fistula, Lingual Implant Placement

A 70-year-old woman has had a fistula buccal to the tooth #26 for more than 7 months (Fig.1).  The buccal plate is concave (Fig.2 *) and must be defective due to the fistula.  Therefore after extraction of the most likely fractured tooth (no Antibiotic, Fig.3 after RCT), start osteotomy in the lingual plate with 1.2 mm drill, followed by PA.  Prepare both straight and angled 1-piece implants
