Premolar Extraction for Molar Implant

A 62-year-old man has sudden severe pain associated with the tooth #4.  Since the edentulous space at #3 is narrow (Fig.1,3), an implant is placed distal to the site of #4 (after extraction) as a molar (Fig.2-4).  Use Lindamann bur to remove the bone in the distal wall of the socket prior to osteotomy (Fig.2 red dashed area).
