Socket Shield Protects Buccal Plate at Extraction
A 73-year-old man (HR) requests restoration at #2-4 (#3 lost crown). To prevent buccal plate fracture associated with extraction at #4, the buccal portion of the root will be preserved (socket shield, surgical bur block) after buccal sulcus incision. Still prepare proximators for extraction. Granulation tissue is going to be removed, followed by Clindamycin. Since there are two canals, start osteotomy using Magic Expander (ME) 3 mm in the palatal portion of the socket apex (oval) for 2 mm. Insert a 4 or 4.5x11 mm dummy implant and take PA to determine placement level. Osteotomy at #2 will begin with Magic split, followed by ME for 9 mm. Use drills if bone density is high. Sinus lift will be done at the sites. An immediate splinted provisional will be fabricated by hand at #2-4.
