Buccal Placement

A 50-year-old woman has multiple restoration (Fig.1).  She complains of upper left sensitivity.  Exam shows that the tooth #14 has deep palatal pocket with purulent discharge.  It appears that the palatal plate is lost (Fig.2 arrowhead).  If it is confirmed after extraction (Clindamycin), an implant will be placed buccal to the center (septum).  Collagen dressing or Osteotape (or preferably PRF if her veins are not too skinny) will be placed against the defective wall, followed by allograft.  Use Magic Expanders or Sinus Lifter.  Her bone is most likely not dense, since long implants are placed at #7-10.
