Osteotomy in Mesial Slope A 64-year-old woman will return for extraction/implant for the upper left 2nd molar with a fistula (Fig.1). Since the roots deviate distal (Fig.2 after extraction), initiate osteotomy in the apical 1/3 of the mesial slope with 1.5 mm drill (Fig.3). Once the lamina dura is penetrated, change trajectory (Fig.4) and extend depth for 15 mm (gingival level). After use of 2 mm drill, insert marked parallel pin for PA.
