Implant to Avoid Abutment Loosening
A 54-year-old man has several restoration with sign of bruxism (Fig.1). The tooth #19 has mesiodistal crack with furca radiolucency (Fig.2 *). To prevent abutment screw loosening associated with heavy mastication, a 6x17 mm tissue-level implant will be placed free hand due to the patient's tight schedule (blue, Fig.3,4,5). Pink: abutment; green: restoration which will be seated partially in the implant to reduce abutment loosening. To reduce possible neuropathy and have freedom to place implant deep, the implant length will be shorter, 14 mm, if the bone is dense. Take photos to show the crack line and buccal swelling. The patient returns for implant 4 months post extraction without bone graft in his home country with bone loss (Fig.6). In the presence of the accessory canal (Fig.7 *), a 5x10 mm implant is safe to be placed.
