A 5x12 mm SM implant with periimplantitis at #30 is removed with a 5/6 mm trephine bur, small elevator and implant driver.  A 3.8 mm Magic drill is used to start a new osteotomy in the lingual wall of the old one.  With the final drill (4.8x13 mm), a 5x11(3) mm Magicore is placed (Fig.1-4).  The thin threads have cut into the bone.  骨质愈合后，钻洞位置更难控制，即使使用导板，坚硬舌侧骨板迫使钻头颊侧偏移（导板其实是盲目手术，不能发觉钻头偏移），只有在直视下，明确目的情境下，即刻重新植入才能准确，发生偏移，可以及时发现，纠正。
