Distolingual Osteotomy
A 40-year-old man (hypertension) without history of cleaning presents to clinic with residual root at #28 (Fig.1). Next visit, SRP will be performed on the right side with extraction of the tooth #1; PA to be taken for #28. Following SRP on the left side at next appointment, extraction and implant (IS) will be done for the tooth #28. Because of the Mental Loop (buccal, red dashed line) and the distally curved apex of the tooth #27 (*), the initial osteotomy will be established distolingual aspect of the socket for ~ 4 mm past the bottom of the socket. Take PA immediately and change the trajectory if needed.
