Ball Abutments for Lower RPD

A 62-year-old woman has failed implant at #27 for the 2nd time.  The crowns at #20 and 21 are loose and non-salvageable (Fig.4-6).  Three implants will be placed at #20, 21 and 28 (Fig.1), since the bone height in the lower right posterior region is limited.  Ball abutments will be used to retain the lower RPD.  When a narrow implant is placed at #28, it will invade the Incisive Canal (Fig.2 red circle), which does not cause neuropathy, but hemorrhage.  In contrast, it is safer to place a 2 mm 1-piece implant with ball type after ridge reduction (Fig.3 black triangle).

At #20 and 21 (Fig.5,6), immediate implants (2 piece) will be placed, although there is possibility of place a 1-piece one at #21 (Fig.5).
