Reduce Abutment Screw Loosening

Abutment screw loosening is one of the most common and annoying implant complications.  It affects clinical work flow and creates anxiety among patients and doctors.  There are 2 groups of etiology for screw loosening, uncontrollable and controllable.  The uncontrollable includes bruxism (heavy mastication), severe bone loss (unfavorable crown/implant ratio), partial edentulism (unbalanced occlusion) and no periodontal ligament for implant (buffer/cushion effect).  The controllable is composed of implant placement misalignment (angle, depth).  A retrospective study is conducted to determine whether surgical guide is able to reduce screw loosening.  Twenty and 18  molars were assigned to guide and non-guide groups, respectively.  The same implants were used for all of the cases.  The study shows that 5 episodes (4 screw loosening, 1 screw fracture in two patients) and 2 episodes (2 screw loosening in 2 patients) of complication occur in the guide and non-guide groups.  The incidences are 25 % vs. 10 %.
