No Sinus Lift Due to Thick Sinus

A 79-year-old woman has subgingival symptomatic caries at #14 and residual roots at #4 and 21 (Fig.1). Because of bilateral thick sinus membrane (Fig.2 *) with nasal obstruction, immediate implants at #14 and 4 will be placed without sinus lift (Fig.3,4). To obtain primary stability, a long implant will be placed at #21 (15 or 16 mm), while a narrower implant (3 mm 1-piece, Fig.6)) is safer than 3.8 mm one (2-piece, Fig.5) in term of preventing periimplantitis (buccal thread exposure). Impression has been taken for guide at #14.
