Internal or External Sinus Lift?

A 43-year-old man has ~ 2 mm bone at #3 (Fig.1).  To reduce sinus membrane perforation and abutment screw loosening, a shorter, but larger Magicore will be used (5x7(3)) if an implant is to be placed at the same time of bone graft.  Since the crestal bone is loose (Fig.2 *), internal sinus lift is feasible with Magic Sinus Lifter.  Flap surgery will make it easy to place bone graft to increase crestal bone height as well as bone volume surrounding the neighboring teeth.  A 6-month membrane will be used for guided bone regeneration (GBR).  A spacer will be placed to keep the space and replace a flipper the patient is wearing now.  The spacer is favorable for wound healing.  PRF membranes will be prepared (x3).  External sinus lift seems to be safer (Fig.3 arrow).
