Proximal Clearance

Place bony anchor at #14 after seating the guide and the 1st 3 steps of drill sequence.  Start osteotomy at #13: 2.2 mm drills all the way to 11.5 mm; take PA to confirm the trajectory and then follow normal sequence; place fixture anchor at #13.  Next osteotomy at #15 and then place fixture anchor.  Last remove #14 bony anchor and begin osteotomy.  10.5 mm offset in Fig.11 is an error.
