PA is taken when a 2.0 mm pilot drill makes an initial osteotomy at the depth of 8 mm (Fig.3); the distance between the distal surface of #4 and the center of the osteotomy is equivalent to that of the design (Fig.1).  In fact a surgical stent is used.  The final depth of the osteotomy is then adjusted at 11 mm.
