The retained deciduous tooth (lower right 2nd molar, Fig.1 T) has buccal apical infection and recession.  In spite of the infection, the large-rooted deciduous tooth keeps the strong buccal plate.  In contrast, an implant to replace the tooth cannot be too large.  Otherwise periimplantitis will occur.  The buccal extended provisional may be unable to prevent the postop bone resorption.
