Septoplasty
A 64-year-old man has pain at night, possibly associated with vertical fracture of the mesial root of the tooth #30, 1.5 years post cementation at #31 (Fig.1,2 (2 weeks post #7 implant placement). After extraction without antibiotic socket treatment, use medium Rongeur to even the irregular septal crest (Fig.3 red line) prior to using 1.6 mm pilot drill. The initial depth will be 13 mm. PA will be taken to confirm the apical position relative to the Inferior Alveolar Canal. An implant is placed at the distal and septal crestal level with bone graft in the distal socket, while the implant is well below the the mesial crest. The implant should be not too big. Make sure that the larger abutment does not touch the mesial crest. Since the entrance to the mesial socket is smaller than that of the distal, bone graft is placed in the mesial socket prior to abutment placement.
